• You MUST send your passport along with your application as the Visa is placed IN the passport. Visa is issued at the discretion of the Embassy.
• Please submit a signed passport valid for at least 1 year + 90 days.
• If posted in their parent's passport, children under the age of 15 do not need a visa. If the child is over the age of 15, he/she will require their own passport and visa.
• Please submit 2 copies of the completed Visa application form, on plain bonded white paper, with the applicant's original ink signature.
• Please provide a copy of your hotel booking (if applicable). Please fax (not email).
• You Must include 2 original recent passport photos. Photos must show a full front view of the face and must be taken against WHITE PLAIN background. Otherwise visa will not be processed. Do not attach photos with staples please.
• Please, be advised that as of November 24th 2005, all visitors to the Czech Republic must be in the possesion of valid health (medical) insurance with the coverage of 30.000 EUR (appr. 50.000 CAD) the minimum valid for the Czech Republic, and for the entire validity period of the visa.
To comply with this obligation, the visitor must be able to give proof of such insurance by presenting the relevant documents to the immigration officers at the port of entry to the Czech Republic. Similarly, applicants for visas necessary to enter the territory of the Czech Republic must present such documents to the consular officer when applying for visas at the consular office of the Czech Republic abroad.
In case your proof is to be in the form of a plastic card, you are also expected to present either an original letter from the insurance company, or the booklet issued with the plastic card. Both documents must contain the above required information icluding all your data.
• A copy of a confirmed personalized itinerary or your ticket, should be included with your application.
• You must submit the following as proof of sufficient funds:
- copy of recent credit card (both sides) or
- original recent bank statement less than 30 days old, showing applicants name, balance and account number or
- original traveler's checks.
